Panel advises against prostate cancer test

MONDAY, May 21 (HealthDay News) -- In a highly anticipated move sure to unleash heated debate, a prominent U.S. government advisory panel is recommending that men of all ages no longer be screened for prostate cancer by undergoing the prostate-specific antigen (PSA) blood test.

The U.S. Preventive Services Task Force, an independent group of medical experts in prevention and evidence-based medicine, said PSA screening results in overdiagnosis of prostate cancer and unnecessary treatment that can leave men impotent and incontinent.

This final recommendation comes seven months after the task force drafted a report giving a "D" rating for the PSA blood test. Previous guidelines had stated that most men should undergo screening beginning at age 50.

"Some may say that by rating the test a 'D' we're taking away the possibility of an informed decision, but we don't want that to be the case," said task force co-vice chair Dr. Michael LeFevre, a professor in the department of family and community medicine at the University of Missouri School of Medicine. "This decision does not preclude a man choosing to be screened."

The task force is the same panel that in 2009 rejected regular mammograms for women in their 40s, after also concluding the benefits don't outweigh the harms.

The new recommendation is published online May 22 in the journal Annals of Internal Medicine.

About 242,000 new cases of prostate cancer will be diagnosed in Americans this year, and about 28,000 will die from it, according to the U.S. National Cancer Institute. More than two-thirds of those deaths occur after age 75, the task force said.

PSA tests -- which measure prostate-specific antigen, a protein produced by the prostate gland -- can detect which men are developing the malignancy. But they cannot discern between cases that will never become life-threatening and those that require treatment, such as surgery, radiation or hormone therapy.

Basing its recommendation mainly on two major trials of PSA testing in asymptomatic men in the United States and Europe, the task force concluded screening may only help one man in every 1,000 to avoid dying from prostate cancer. Up to five in 1,000 men will die within a month of prostate cancer surgery, the panel said, and between 10 and 70 per 1,000 men will suffer lifelong adverse effects, such as urinary incontinence, erectile dysfunction and bowel dysfunction.

Many will also suffer unduly from persistent anxiety, the report said.

Critics of the task force's decision said the group underestimated the PSA test's benefits and overestimated its harms and also overlooked significant methodological flaws in the studies on which it based the "D" recommendation. None of the task force members are urologists or oncologists, making them unqualified to broach the topic, some health experts said.

"I think they're throwing away the baby with the bathwater," said Dr. William Catalona, a professor of urology and director of the clinical prostate cancer program at Northwestern University's Feinberg School of Medicine in Chicago, who wrote an accompanying journal editorial condemning the move.

"There's just no other way to detect prostate cancer early than through PSA testing," Catalona added. "If we were to completely stop PSA testing in all men . . . it would result in countless men dying of metastatic prostate cancer. People are happy to be cured of their prostate cancer, even if they have some side effects."

Dr. Otis Brawley, chief medical officer at the American Cancer Society and an outspoken figure on the pros and cons of cancer screening tests, encouraged men to make their own choice about PSA screening while keeping the risks and benefits in mind. Certain men, including blacks and those with a family history of the disease, are at significantly higher risk of developing prostate cancer, he noted.

"I am hoping this (recommendation) shuts down mass screenings, where men are only told that this will help them, which is stretching the truth," Brawley said. "They're rarely told about the potential for harm and that, for the people running the screening, the [venture] is usually quite lucrative for them."

LeFevre said it remains to be seen whether health insurers will change their coverage of PSA screening based on the task force's recommendation. For one thing, insurers aren't supposed to tailor coverage decisions to USPSTF guidelines, he said.

"If you look at the overall costs of screening, the PSA test itself is very small," LeFevre said. "I'd be surprised to see them take what I call the political risk of backing away from that."

With better car safety, highway deaths falling

If the numbers in a preliminary federal study hold true, U.S. highway deaths continued to plunge last year ? when measured in terms of fatalities per 100 million miles driven - to the lowest level since the government began keeping records in 1921.?

A number of factors have contributed to the decline, which has seen traffic deaths dip 25 percent just since 2005. These include better roads and signage and a crackdown on drunk drivers.? But another significant factor is the continuing improvement in the safety of the vehicles we drive.

By using the latest in computer-aided design technology, manufacturers have learned how to shield motorists and passengers from the deadly forces of even the most serious crashes.? Equally significant are the various high-tech safety devices that are finding their way into the industry?s newest models and which can help motorists avoid accidents in the first place.?

?Our research shows that some of the newest forward collision avoidance systems, such as Volvo?s City Safety, are reducing collisions by as much as 20 percent to 30 percent,? said David Zuby, chief research officer for the Insurance Institute for Highway Safety.

The 2013 model year will see some of the most sophisticated systems yet developed ? and in many cases technologies once limited to only the most expensive models are migrating down to even entry-level vehicles.

It?s been more than 40 years since federal regulations began mandating improvements in automotive safety, with the seat belt being one of the first devices written into law.? In 2011, the National Highway Traffic Safety Administration added electronic stability control ? which is designed to keep a car under control on slick surfaces or when it makes an overly aggressive maneuver ? to the list. And the NHTSA may soon add rear back-up cameras to the list.

Credit ever faster microprocessors, along with the overall drop in the price of electronic hardware, for making it possible to mandate such technology.? But where industry planners routinely resisted new federal regulations in decades past, these days manufacturers are likely to introduce new features ? often as standard equipment ? without pressure from the government.

?There?s an expectation on the part of customers for more and more safety equipment,? explained General Motors designer Bob Boniface.

The challenge these days, he said is to find a place to put all the various controls and sensors. The front ends of some of GM?s most advanced models. are ?covered? with radar and laser sensors, he said, with many new models also introducing advanced video cameras as well.

The latest safety technology roughly divides into two separate categories: active and passive systems.? Passive systems are those designed to reduce injuries if an accident occurs. An air bag is such a system.? Expect to see some models, such as the 2013 Chevrolet Malibu, offer as many as 10 air bags.? The new two-seat Scion iQ microcar is offering the first air bag designed to protect a passenger in the event the vehicle is struck from the rear.

Ford?s is expanding the use of its new inflatable rear seat belts, now found in several models, including the recently redesigned Explorer.? According to just-retired Ford safety chief Sue Cischke, the devices are particularly well-suited for use by the very young and the very old because they can suffer internal injuries from the forces acting on standard seat belts in severe collisions.

The 2013 model year will also see a number of new applications of the most advanced active safety systems, which are designed to prevent accidents.? A wide range of new mainstream products will add forward collision avoidance, blind-spot monitoring and active lane-keeping.? The 2013 Ford Fusion will not only warn drivers that they are drifting out of their lanes but will also help steer the car back into the lane. If the system detects that the driver might also be drowsy, it will sound an alert warning the driver to pull over and rest.

Significantly, many of the most advanced technologies are migrating down-market.? Cross-traffic alert was introduced only a few years ago on the redesigned BMW-7 Series. The system, which uses radar to monitor oncoming traffic as a driver backs out of a parking spot, will now be offered on a wide range of midline models, such as the 2013 Nissan Altima.

The redesigned midsized sedan also features blind-spot monitoring, lane departure warning and other systems as part of an optional technology package.? The trick, explained Nissan product manager John Curl. was to use a single wide-angle rear camera instead of an assortment of much more expensive radar and sonar sensors.?

Subaru will take a similar approach in 2013 with its new EyeSight system, technology it has been tweaking for almost a decade.? The system uses a pair of stereoscopic cameras mounted on either side of the rear-view mirror to look out onto the road ahead.? It?s a lower-cost alternative, the maker claimed, compared to the radar-based safety systems currently in use by luxury makers such as Mercedes-Benz and BMW.

According to IIHS research, the new technologies are paying off.? The basic Mercedes forward collision warning system has reduced accident rates by approximately 6 percent. And the maker has introduced a more advanced version of what it calls Distronic, which can apply the brakes if it senses the driver is not reacting in time to a potential collision.

Cadillac is now working on a prototype system that could also steer a vehicle around an obstacle without a motorist?s involvement.? And a number of makers ? as well as tech giant Google ? are working on fully autonomous vehicles that some proponents believe could be ready for sale before decade?s end.

The good news is that even the features now on the road are saving lives, but IIHS research chief Zuby stressed that because the typical American car remains in use for a decade or longer, it will be close to 2020 before most vehicles in the U.S. fleet have advanced technologies such as electronic stability control, let alone collision warning and blind spot detection.

On the other hand, he added, ?As older vehicles retire and these technologies come into a broader segment of the fleet, we can expect to see the death rate continue to decline.?

    Blind Chinese activist leaves Beijing for U.S.

    BEIJING (Reuters) - China allowed a blind legal activist, Chen Guangcheng, to leave a hospital in Beijing on Saturday and board a plane bound for the United States, a move that could signal the end of a diplomatic standoff between the two countries.

    Chen's escape from house arrest in northeastern China last month and subsequent stay in the U.S. embassy caused huge embarrassment for China and led to a diplomatic rift while U.S. Secretary of State Hillary Clinton was visiting Beijing for talks to improve ties between the world's two biggest economies.

    The U.S. State Department said he was en route to the United States, along with his wife and two children. He boarded a United Airlines flight bound for Newark.

    China's Foreign Ministry limited its commentary to an acknowledgement that Chen had left the country.

    "Chen Guangcheng is a Chinese citizen. China's relevant departments have handled the procedures for exiting the country in accordance with the law," the ministry said in a faxed statement to Reuters.

    New York University said in a statement on Saturday that Chen would study as a fellow at its School of Law.

    "I am very happy to receive the news that Chen Guangcheng is on his way to the U.S. I look forward ... to working with him on his course of study," said Jerome Cohen, co-director of the U.S.-Asia Law Institute at New York University School of Law.

    State news agency Xinhua said earlier that Chen had applied to study in the United States under legal procedures. The Foreign Ministry said this month that Chen could apply to study abroad, a move seen as a way of easing Sino-U.S. tensions on human rights.

    Chen's friend, Jiang Tianyong, cited the activist, one of China's most prominent dissidents, as saying that he and his family obtained their passports at the airport hours before he was due to board a flight.

    "I'm obviously very happy," Jiang said. "When he boards the plane, he can finally say: 'I'm free'. At the same time, I feel a sense of regret because such a large country like China can't even tolerate a citizen like him to exist here."

    A statement by State Department spokeswoman Victoria Nuland struck a conciliatory note, saying Washington was "looking forward" to Chen's arrival.

    "We also express our appreciation for the manner in which we were able to resolve this matter and to support Mr. Chen's desire to study in the U.S. and pursue his goals," it said.

    U.S. President Barack Obama's administration had feared a standoff over Chen's fate could sour already strained ties with China and generate criticism of Obama's policies. Beijing has accused Washington of meddling in its affairs in the case.

    Chen's abrupt departure for the airport came nearly three weeks after he arrived at the Chaoyang Hospital from the U.S. embassy, where he had taken refuge after an escape from 19 months under house arrest in his home village.

    Chen, 40, who taught himself law, was a leading advocate of the rights defense movement. He gained prominence by campaigning for farmers and disabled citizens and exposing forced abortions.

    He was jailed for a little more than four years from 2006 on what he and his supporters say were trumped-up charges designed to end his rights advocacy.

    He had accused Shandong officials in 2005 of forcing women to have late-term abortions and sterilizations to comply with strict family-planning policies. Authorities moved against him with charges of whipping up a crowd that disrupted traffic and damaged property.

    Formally released in 2010, he remained under house arrest in his home village, which officials turned into a fortress of walls, security cameras and guards in plainclothes guards.

    POLICE AT THE AIRPORT

    United Airlines flight UA 88 departed around 5.50 p.m. (0950 GMT), with police officers and plainclothes officers following passengers down the mobile corridor leading to the plane's door.

    The cabin crew waited for passengers to take their seats before closing the curtain to the front section, where the business class seats were located, a Reuters witness said.

    Chen had earlier told Reuters he was at the airport along with his wife, two children and hospital staff and he believed he would be put on a flight to the United States.

    Two police cars were stationed below the walkway to the plane, and about 10 security officials in plainclothes circulated around the airport.

    Passengers at the gate to Chen's flight appeared not to know that he would be on the same flight.

    "If our country is a body, his plight is like a sickness that in the future will help the body to protect and strengthen itself," said Xi Jingwen, who was awaiting to board a flight to the United States, when asked about Chen Guangcheng.

    Chen's confinement, his escape and the furor that ensued have made him part of China's dissident folklore: a blind prisoner outfoxing Communist Party controls in an echo of the man who stood down an army tank near Tiananmen Square in 1989.

    The Chen case comes at a tricky time for China, which is engaged in a leadership change. The carefully choreographed transition has already been knocked out of step by the downfall of ambitious senior Communist Party official Bo Xilai in a scandal linked to the apparent murder of a British businessman.

    On a number of occasions in recent years, authorities have relented to diplomatic pressure and allowed high-profile dissidents to leave China, knowing that its most vocal critics are effectively neutralized once they leave and are without support of friends.

    At times, Beijing has appeared to use these deals as bargaining chips in broader diplomatic negotiations or to blunt criticism of its human rights record.

    Chen's supporters, however, welcomed his departure, saying he had indicated that he would like to return to China.

    "I even told him ... that he has to do a repeat of him scaling walls. If not, we wouldn't be able to believe it," Nanjing-based activist He Peirong said of her earlier conversation with Chen. She was one of six activists who drove Chen from Shandong to Beijing after his escape.

    Phelim Kine, senior Asia researcher at New York-based Human Rights Watch, said "getting Chen Guangcheng and his family on a plane is the easiest part of this saga.

    "The harder, longer term part is ensuring his right under international law to return to China when he sees fit," Kine said in an emailed statement.

    Kine urged Western countries to ensure that Chen's relatives, friends and supporters secured due protection.

    The U.S. embassy had earlier thought it had stuck a deal to allow Chen to stay in China without retribution, but that fell apart as Chen grew worried about his family's safety. He changed his mind about staying and asked to travel to the United States.

    Human rights are a big factor in relations between China and the United States, even though Washington needs China's help on issues such as Iran, North Korea, Sudan and the global economy.

    The village of Dongshigu, where Chen's mother and other relatives remain, is still under lockdown.

    Chen's nephew, Chen Kegui, was denied his family's choice of lawyers on Friday to defend a charge of "intentional homicide", the latest in a series of moves to deny him legal representation, and underscores the hardline stance taken against the blind dissident's family.
